You now have the option of registering online before your visit. Pre-registration will save you valuable time and allow the staff to give you more efficient service upon your arrival. Please enter the secure Patient Online Clinic portal to complete your insurance, demographics and health history prior to your appointment. You can also request an appointment time, communicate with the staff and refill your prescriptions. You will need to have your insurance cards and medication list readily available to complete registration. To insure that your registration will be processed prior to your appointment, complete your online registration 24 - 48 hours prior to your arrival. Allow about 15 minutes to complete this process.
